Stages of Care

Arcadia Chiropractic prides ourselves on only accepting patients for care that we can help. We regularly tell people that if we can help them, we’ll tell them. If we can’t, we’ll make sure you get to the right person who can.

If we do accept you for care there are generally three stages. The first stage of care has a focus on relieving your symptoms as soon as possible. The next stage addresses patterns in daily activities and changing structural imbalances. The final stage demonstrates the effectiveness of maintaining proper health so that the issue stays resolved.

Relief Care

The first stage is often called Acute or Relief care and is for most patients. The focus in the beginning is on providing you with the greatest amount of symptom relief as soon as possible. Since everyone is different, some individuals heal quickly while others take weeks to get the desired effect. We primarily focus on techniques and therapies that maximize effectiveness in alleviating pain and discomfort. Our goal is to allow you to participate in “activities of daily living” with minimal distress. For this to happen, the doctor often prescribes at home therapies for the patient to do starting the first night.

Corrective Care

The next stage of care focuses on correcting the problems from a more functional and structural standpoint. Our goal is to increase you bodies functional ability by promoting the right kind of movement through your joints, muscles and spine. Since the patient is the one healing, we all work together as a team to accomplish these goals. It is extremely important for the patient to follow the treatment guidelines during this stage because a lot of the healing is happening outside of the office. Consistency is the key factor in making appointments, maintaining home exercise plans, and all other recommendations. People eventually realize that even though the pain and discomfort has subsided, the real healing and correction of functional issues needs to continue.

Wellness Care

A mentor of mine, Dr Guy, said that Wellness Care is about changing. I am of the opinion that life is far more than just getting rid of or preventing disease. Life is a process of growth and development that changes from one day to the next. Physically, emotionally, spiritually, intellectually and in our relationships, we are different than we were at age two (and even yesterday, for that matter). The process of life, the human experience, is one of maximal growth and development in each of these areas. We need to be prepared to design and develop our lives fully. This is wellness care.

The Adjustment

As a Phoenix Chiropractor, Dr. Dominic promotes the cornerstone of his treatment to be the spinal adjustment. As mentioned before, these adjustments are used to help a variety of different conditions and health issues. Since the foundation of our health is determined by how well our nervous system is functioning, Chiropractic Doctors help you preserve that essential function.

Massage Therapy

Deep Tissue Massage

One of the most common therapeutic and restorative massages. “Deep tissue” refers to the muscles that lie beneath the superficial layer of muscles, which bear the most strain during strenuous activity and repetitive motion. These muscles often become overused, tight, and suffer what we often refer to as “knots,” pain and soreness. A deep tissue massage will use slow, deep strokes against the grain of the muscle to release knots and relieve tension.

Corrective Massage

Aims to massage your muscles in order to bring your body into more proper alignment, similar to chiropractic work. Tension and strain can restrict range of motions and throw movements out of balance. The therapist will listen to your pains and use soothing massage to help make alignment changes. Regular corrective massage will re-educate muscles to perform in balance.

All of our licensed Massage Therapist’s use a number of soft tissue therapy techniques which help to reduce pain, muscle hyper-tonicity and spasm. Some of the techniques we employ are trigger point therapy, mysofacial release, proprioceptive neuromuscular facilitation, acupressure point therapy, and active tension release. The need and benefits of these therapies vary from patient to patient. Its important to see that if your specific condition requires massage, Dr. Dominic will prescribe it for you and incorporate it into your treatment plan.

Adjunctive Therapies

Physical Therapy

Once we determine the correct cause of your ailments, it is important to help maintain proper joint motion. As a patient who was a dentist once told us, all of our therapy stretches and exercises are meant to do one thing: help support proper joint hygiene. Since the primary focus is on consistent mobility, we encourage a proactive approach towards healing. That is why Dr. Dominic creates specific “Home Protocols” for each of his patients to do both in the office and at home. Adding a physiotherapy regime to your health program helps to speed your recovery by stabilizing your “Core Muscles” and improving your body’s “biomechanics”. Your muscles are there for support, so why not use them to the best of your ability.

Spinal Decompression

A new technology used to treat disc injuries in the neck and the low back. This treatment option is very safe and utilizes FDA cleared equipment to apply distraction forces to spinal structures in a precise and graduated manner. During treatment, the distraction is offset by cycles of partial relaxation. The technique of spinal decompression therapy helps to unload pressure on the spinal disc due to distraction and positioning. This movement gently separates the vertebrae from each other, creating a vacuum inside the discs, which pulls in the protruding disc bulge. The "vacuum effect" of decompression is also known as negative intra-discal pressure. The computer assisted cycles of decompression and partial relaxation promote the diffusion of water, oxygen, and nutrient-rich fluids from the outside of the discs to the inside. These nutrients facilitate the healing of torn and degenerated disc fibers. For the lower back, the patient lies fully clothed on his/her back. Next, a set of nicely padded straps comfortably fit around the waist and lower chest. Many patients feel instant relief while they are on the table because all pressure is being relieved off of the nerve. Non-Surgical Spinal Decompression is very effective in treating Sciatica, Bulging or Herniated discs, pinched nerves in both the neck and back as well as radiating arm and leg pain.

Kinesio Tape

As an Athletic Trainer, Dr. Dominic utilizes sports training and Kinesio Taping with a majority of his patients. You probably first saw Kinesio-Tape on Kerri Walsh’s shoulder during the Olympics in 2008 or on Lance Armstrong’s Knees in the 2009 Tour de France. This sports specific tape was first designed by a Japanese Chiropractor called Dr. Kenso Kase in 1997. Dr. Dominic started using it with his athletes in 2002 and then with “real people” in 2006. Since then he has utilized its specific ability to aid the healing process by encouraging normal body motion.

Workplace Ergonomics

Since you spend half of your life at work, it’s important to have the correct type of environment for you. While working as an ergonomic assessor, Dr. Dominic realized that most workplace ergonomics can be corrected by the person working there. That is why he has designed a specific tutorial to help people set up their own ergonomic workstation. This ergonomics training go’s hand in hand with our philosophy on empowering  patients to take control of their own health.

Doctor Consultation

This is the time when you sit down with the doctor and discuss your specific health-related concerns, as well as possible options. We take great care in treating the hole person and look forward to getting to know more about you.

History and Physical Examination

The doctor takes great care in determining what kind of actual problem you have, and if he is the correct physician to help you. He does this by using a number of different specialized tests such as orthopedic, neurologic and proprioceptive. These tests are performed to determine the severity of the condition and what tissues are affected.

X-ray Studies

Some specific conditions require the use of x-rays to not only confirm the diagnosis but to also rule out other more serious problems that may be developing. These x-rays are only taken when they are absolutely necessary and only of the affected areas. We use the latest procedures to reduce exposure and clearly evaluate the problem.

Treatment

Once finished with the examination and all appropriate studies, patients may be provided with their initial treatment that day. This may include the aforementioned adjustments along with physical and/or soft tissue therapies. If additional testing is required, treatment will be provided during the second visit.

Home Instructions

Prior to leaving, the doctor commonly gives patients specific instructions on certain activities that should and should not be done at home. This may include the use of ice or heat, the correct positions, as well as home exercises and/or stretches.

Generally, the doctor recommends a follow up visit within 1-2 days after your initial visit to review your diagnosis and treatment options. Dr. Pisaro is an alumnus of Palmer College of Chiropractic West and Chico State University. He is originally from the San Francisco bay area where he spent the last 16 years immersed in numerous branches within health care. He is also the creator of S.P.E.A.K. (Student Patient Education Awareness Knowledge), which is the community outreach program at the three Palmer colleges that supports and empowers students to develop into competent, passionate doctors of chiropractic for the ultimate improvement in the health of the world.

Dr. Pisaro’s vast clinical experience in Chiropractic, Massage and Rehabilitation enables him to help a wide variety of patients with difficult conditions. As a CPR / FIRST AID INSTRUCTOR, Karate Teacher, and soccer coach, he empowers his students to discover their true potential.

Dr. Pisaro continues to complete advanced training in a number of health care related fields to increase the effectiveness of his treatments. He is a certified practitioner of Kinesio Tape which is commonly associated with elite athletes in the Olympics. His extensive training in Gonstead, Flexion Distraction, and Pro-Adjuster allows for a more precise and skillful adjustment.

Since Dr. Pisaro has been practicing in Arizona for over eleven years, he has had the opportunity to assist his community in a number of ways. He was proud to be on the Board of Directors for UMOM, a family based homeless shelter in Phoenix. MAYO Clinic in phoenix asked him to speak at the annual symposium for the otolaryngeal and esophageal treatment. He is also currently teaching numerous classes on topics from Trigger point massage to functional nutrition and Fitness boot camps which encourages all of his patients to live a wellness based lifestyle.
